DIRECT PATIENT CARE
- Admission and discharge of geriatric patients.
- Consults and coordinates with health care team members to assess, plan, implement and evaluate patient care plans for geriatric patients.
- Assesses the geriatric patients ability to do routine daily tasks
- Assists patients with such basic needs as dressing, eating, and bathing, and encourages patients to do things for themselves to retain feelings of independence and self-esteem
- Recognizes and manages geriatric syndromes common to older adults, including: cardiovascular, respiratory, gastrointestinal, urinary, hematological, musculoskeletal, endocrine, neurological, psychiatric/psychosocial, integumentary, sensory and pain problems.
- Prepares equipment and aids physician during examination and treatment of geriatric patient.
- Facilitates older adults active participation in all aspects of their own health care
- Promotes the desirability of quality end-of-life care for older adults.
- Assisting elderly patients suffering from terminal illnesses like diabetes, Alzheimers disease, high blood pressure, cataract, cardiovascular diseases, arthritis and more.
- Assess the cognitive skills and mental status of their patients.
- Observe, understand, and manage peculiar old age health concerns like incontinence, insomnia, loss of strength and appetite, sexual issues as well as mobility challenges.
- Help to educate the families and caregivers of their patients on the best ways to care for them and keep them safe.
- Monitor for signs of elder abuse.
- Maintain the personal hygiene of the patients, including bathing, care of mouth, back, nails, hair etc.
- Care of pressure points as needed.
- To assist the patient in elimination, offering and removing the bed pans and urinals.
- Bed making.
- Assist in feeding the weak and debilitated patients.
- Ensuring ordering of therapeutic Diet through computer requests as instructed by consultant for patients.
- Coordination with physiotherapy, ambulation and rehabilitation departments.
- Carry-out patients teaching and demonstration according to the need.
- Counselling the patients and relatives.
- Care of the dying and dead.
- Administration of medicines, injections, infusion and transfusion to the patients.
- Observing, recording and reporting of vital signs e.g. T.P.R. and blood pressure.
- Carry out technical procedures, such as NG intubation, gastric lavage, oxygen therapy, etc.,
- Dressings and irrigations, Enema, Catheterisation, hot and cold applications, suction etc.
- Collecting, labelling and dispatch of specimens.
- Preparation for and assistance in clinical tests and medical/ surgical procedures.
- Observation, recording and reporting of all procedures and tests.
- Escorting serious patients to and from the department/ wards.
- Patient education in case of discharge.
- Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms
WARD MANAGEMENT
- Handing over and taking over charge of patients and ward inventory in each shift.
- Maintenance of therapeutic environment in the ward.
- Keeping the ward clean and tidy.
- Maintaining the cleanliness of dressing trolleys, cupboards, apparatus, etc.
- Care of clean and soiled linen.
- Disinfection of linen, beds, and fumigation of rooms etc.
- Preparation of rooms, trolleys and sets for procedures.
- Indenting of surgical supplies
- Maintaining interpersonal relationship with patients, relatives and health team members
- Orientation of new staff/ students
- Demonstration and guidance to student nurses
- Participation in staff education and staff meetings
- Participation in professional activities
- Demonstration and supervision of domestic staff
- Report about the medico legal cases if any admitted in the ward
- Any other duty that may be assigned by ward incharge/supervisor from time to time.
- Reporting any untoward incidents to ward in-charge.
- Any other duties as assigned by the Management from time to time.
